Mounjaro Price in Japan 2026|How Much Does It Cost in Kyoto?
If you are visiting Japan and need to continue Mounjaro® (tirzepatide) treatment, one of the most common questions is:
“How much does Mounjaro cost in Japan?”
There is no single nationwide self-pay price for Mounjaro in Japan. Prices for treatment outside public health insurance can vary between clinics.
At Kajiyama Internal Medicine Clinic in Kyoto, Mounjaro is available to international travelers after a face-to-face medical consultation, when the doctor determines that treatment is medically appropriate. Our current self-pay prices are shown below.
How Mounjaro is sold in Japan
Mounjaro is injected once weekly.
At our clinic:
1 pen = 1 week
1 box = 2 pens = 2 weeks
2 boxes = 4 pens = approximately 4 weeks
Mounjaro is sold by the box. The price per box becomes lower when several boxes are purchased at the same visit. [1]

For a typical 4-week supply (2 boxes / 4 pens), the current medication prices at our clinic are:

For travelers who need a longer supply, the price per box decreases according to the total number of boxes purchased at one time.

All prices include tax.
The discount tier is determined by the total number of boxes, even if different doses are combined.
For example:
1 box of 2.5 mg + 3 boxes of 5 mg
= 4 boxes in total
Therefore the 4–5 box price applies:
2.5 mg: ¥8,000 × 1
5 mg: ¥14,000 × 3
Total medication cost: ¥50,000.
How much Mounjaro can a traveler purchase?
At our clinic, a patient may purchase up to 12 boxes at one visit, depending on the doctor’s assessment.
Because each box contains two weekly pens, 12 boxes corresponds to up to 24 weekly doses, or approximately six months of treatment.
The amount prescribed is not guaranteed in advance. The doctor will review your current dose, treatment history, health condition and other medications before deciding whether treatment and the requested quantity are appropriate.

Are there any additional fees?
The initial consultation fee is ¥2,200.
If you need an English medication certificate for international travel:
English medication certificate: ¥3,300
The certificate is free when your medication purchase is ¥40,000 or more.
If you plan to take Mounjaro back to another country, please also check the medication import requirements of your destination country.
Can tourists get Mounjaro online?
No.
For international travelers, our clinic provides Mounjaro treatment through face-to-face consultations only. We do not provide online Mounjaro consultations for overseas travelers.
You can make an appointment online or through LINE, or visit the clinic directly during reception hours.
Because our reception staff have limited English-speaking ability, telephone appointments in English are not available. During the medical consultation, we can use a translation application to assist with communication.
Is Mounjaro approved for weight loss in Japan?
This point is important.
In Japan, Mounjaro is approved as a treatment for type 2 diabetes mellitus. The Japanese regulatory authority PMDA lists Mounjaro 2.5 mg through 15 mg as tirzepatide products.
The same active ingredient, tirzepatide, is marketed as Zepbound® in Japan for obesity treatment under specific approved criteria.
Therefore, when Mounjaro is prescribed for weight-management purposes at our clinic, it is provided as self-pay treatment outside the approved indication of Mounjaro in Japan.
What should I bring to the clinic?
If you are already using Mounjaro outside Japan, it is helpful to bring:
- your current dose
- your medication box or pen, if available
- a photo of your prescription
- your most recent prescription or medical record
- the date of your last injection
- information about other medicines you take
- information about previous side effects
A prescription from your home country does not automatically guarantee a prescription in Japan.
The doctor will decide whether Mounjaro can be prescribed after the consultation.
Can I walk in without an appointment?
Yes.
International travelers may visit our clinic directly during reception hours, although making an appointment through the website or LINE can reduce waiting time. [1]
